Volunteer: Lead for Community Health Champions Meeting

Position Description:

This honorary volunteer positon is for one year. The individual will lead the weekly Champions meeting and will oversee the continuity for the Community Health Volunteer program. The opportunity will help develop leadership competency including team building, meeting coordination, and program coordination skillsets.

There is no age requirement for this portion, however, the applicant must exhibit maturity and basic leadership and business office skills to apply. They will be coached by the WFC leadership for their professional and personal development.

Volunteer: EHE Communications Team

This team of volunteers will set up, and host community health education events to promote health equity understanding in various communities across Chicagoland. The volunteers will help organize, create flyers, post on social media, and manage the 5 types of community health events: 1. P.E.P. Talks Webinar series; 2. Health Equity Interactive Workshops; 3. Health Education Talks; 4. Brown Bag Rx & Health Screening; 5. Benefits Clinic & Health Coverage. These events will take place in three different locations in Suburban Cook and Collar counties over the period of next 7 months. 

Volunteers will learn the health equity language, be introduced to health equity measures, develop skills to work with social determinants of health, and build competency to work with health disparities.  Top-performing volunteers will be recognized at the State level.

This clinic provides care and resources to uninsured and underinsured patients.

ROLE AND RESPONSIBILITIES

The primary function of the community health volunteer (CHV) will include helping understand various social determinants of health factors for patients/clients and connecting patients to resources. The CHV will record all social information into the correct online database and determine if the patient needs further support. Specific responsibilities include:

Recording of patient demographic information into Athena EMR and/or the CM99 database Assisting with patient vitals/intake Knowledge of community/social work programming and resources for specific patient cases Handling and dealing with sensitive/personal patient information
